Please ensure you post, fax or email a valid Referral, the Patient Registration form, Health Questionnaire, Privacy Policy, any previous scans or blood results and current medication list, prior to your appointment. This enhances practice efficiencies and assists in the smooth running of your appointment. If you choose to use e-mail, please note that it is not a secure method of transferring private information. You do so at your own discretion.

If you are not able to get this information to us ahead of time, please arrive half an hour before your appointment.

Yes, a Referral from your General Practitioner or Specialist is required.  

While every effort is made to begin your appointment on time, patients with special needs or the frail and elderly can cause unexpected delays. We ask for your consideration and patience.  

All appointments with a Medicare item number have a Medicare rebate, reception staff can let you know the amount.  

The contents of your medical file will not be released without your signed consent. Your medical records are handled with the upmost care for your privacy. Only healthcare professionals involved in your care will access your medical information. Vascular Health Group has a strict patient confidentiality policy.

Yes, payment is required at the time of consultation.

Yes, if you have Extras Cover

Yes, all compression stockings are measured and fitted by our nursing staff. If we have recent measurements on file, you may purchase stockings from our reception staff at any time.

No, our Rooms are NOT located in the main hospital. We are in a separate building across the road. Level 1, 21 Burwood Road, Hawthorn, VIC, 3122.

We are located across the street from Epworth Hawthorn. Parking is available at the rear end of the building.

 

Hawthorn Train station is located across the road (Belgrave, Lilydale and Alamein Lines).

There is also a Tram Stop situated nearby. Route 75, Stop 25 (St James Park) along Burwood Road.
